Q1: IMHO you should be fine with the extended license.

Q2: The license agreement says each usage is limited to a single project. If you are making two separate videos that will be sold separately that’s probably two different projects. Here’s a link to the knowledge base: http://support.envato.com/index.php?/Knowledgebase/List/Index/70/licensing

Q3: As already pointed out, you can download a watermarked preview.

Q4: There is no such limitation, as far as I know.

How many tracks did you compose and publish in 2012?

For me that’s 46 original tracks (a good share went to AJ) excluding sound-alikes instrumentals and production work for clients.
